Why Enroll Your Specialty Pharmacy in the Prolia® Co-pay Program?
When your pharmacy registers for the Prolia® Co-pay Program, you can accept the Prolia® Co-pay Card as part of your transactions and help eligible patients enroll, too. Once they're enrolled, they can save on each dose of Prolia®, download and submit essential forms, and re-enroll every year on the co-pay site.
1. Enroll Your Pharmacy
Fill out a form on this site to take part in the Prolia® Co-pay Program, and a customer service agent will call you back within 1 business day to complete the process. (You can also complete the process by calling 1-844-369-9962, Monday through Friday, 9 am to 8 pm ET.) If your pharmacy is already enrolled, you can move on to step 2.
2. The Patient Enrolls
Patient is prescribed Prolia®, receives a Prolia® Co-pay Card from their HCP, and enrolls in the Prolia® Co-pay Program (if eligible).
3. Confirm Eligibility
Your pharmacy calls the patient to confirm program eligibility and verify that the completed Privacy Notice and Patient Authorization Form has been sent to the Prolia® Co-pay Program. If that form has not been sent, the patient should call 1-844-369-9962 to review and accept the terms and conditions.
Remember that patients need to enroll and agree to the Privacy Notice and Patient Authorization themselves.
4. Accept Payment
Upon confirmation, appropriate co-pay may be collected and account info (debit card or RxBIN and member ID) may be used to cover remaining out-of-pocket costs.
5. Ship Medication
Your pharmacy ships Prolia® to site of care or directly to patient. Transactions are processed at time of shipment until card limit is reached.
6. Check on Status
When you sign in at ProliaSupport.com, you can check on important details related to each patient.
